We support older Australians through Home Care Packages, private and community-based aged care services. Care is delivered by experienced Support Workers, with Registered Nurses available if required to provide clinical oversight, care planning and escalation as needs change.

We work in partnership with a registered aged care provider to provide high-quality in-home aged care to older Australians receiving Home Care Packages across Ipswich, Brisbane, Logan and the Scenic Rim.
We work with individuals living independently, families seeking trusted home-based support and health professionals such as GPs, hospital discharge teams and aged care case managers.
Our services range from personal care to specialised clinical nursing, delivered with compassion, dignity and professionalism.
Aged Care Services
We support older Australians with a comprehensive range of clinical and personal care services, tailored to individual needs and focused on safety, dignity and independence at home. Supporting people to live safely and comfortably at home and in the community:
Personal care & hygiene: Showering, grooming, dressing, toileting, mobility and daily routines.
Domestic assistance & meal preparation: Cleaning, laundry, lawn care, basic outdoor tasks and meal preparation.
Medication support: Medication prompting, administration, monitoring and education in line with care plans and scope of practice.
Community access, companionship and routine support: Support with appointments, shopping, social outings and community activities, delivered through consistent, relationship-based care.
Registered Nurse clinical care: RN-led care matched to individual health needs and clinical complexity.
Wound & skin integrity care: Assessment, treatment and ongoing wound and skin management.
Chronic condition and diabetes care: Ongoing support for chronic conditions, including blood glucose monitoring and insulin administration.
Complex nursing care: PEG feeding, catheter care, tracheostomy care and suctioning, injections and wellness checks.
Palliative and end-of-life care: Compassionate care focused on comfort, dignity and family support.
Clinical monitoring and health liaison: Ongoing monitoring, documentation, escalation and liaison with GPs, hospitals and allied health professionals.
